Supporting Document:
1. Society registration certificate
2. Renewal of Society registration certificate (Latest) OR
3. Trust Deed
4. By-Laws of the society/trust
5. Rental agreement/ Sale Deed/ Lease Deed
6. Pictures (minmum 5) indicating that building is Senior Citizen friendly
7. Audit reports of preceding 2 financial years (Audited statement of accounts)
8. Annual Reports of preceding 2 years describing service activities of the organisation
9. List of beneficiaries which includes their names, permanent addresses and their incomes
10. List of employees which includes their names, designations and qualifications

Service Charge (Free for Online Submission) : Rs 100

1. Applicant fills the appeal form
2. Department will process the application
